Juvenile rheumatoide Arthritis

Syn: juvenile chronische Arthritis (JCA)

Vork: meist 3.-8. Lj.

Etlg: 5 Subtypen, u. a. systemische IRA = Morbus Still

Histr: Georg Frederick Still (1868-1941), Pädiater aus London, ist Namensgeber

CV: Eine Streptokokkeninfektion besitzt keine pathogenetische Bedeutung

KL: - Beginn mit septischen Fieberschüben

- generalisiertes, kleinfleckiges, blassrotes, flüchtiges Exanthem (50% d. F.)

- symmetrische Polyarthritis (rel. oft mit initialer Monarthritis des Kniegelenks)

- Splenomegalie mit Lymphadenopathie (Felty-Syndrom)

- Polyserositis, Peri- und Myokarditis (aber keine Endokarditis)

- Uveitis

- bandförmige Hornhautdegeneration

Bed: pathognomonisch

Bef: weißes Band quer über die Kornea ziehend

Verl: von den Hornhauträndern ausgehend und zur Mitte fortschreitend

Pa: Kalzifikationen

Kopl: Katarakt

Di: - ANA und RF negativ

- hypochrome Anämie

- Thrombo- und Leukozytose

- alpha1- und alpha2-Globuline in der Eiweißelektrophorese erhöht

- Ferritin stark erhöht

- BSG erhöht

So: Morbus Still des Erwachsenen

Syn: adulter Morbus Still

Engl: adult onset Still´s disease

Hi: dyskeratotische Keratinozyten in der oberen Epidermis und im Stratum corneum mit einem neutrophilenreichen dermalen Infiltrat
